Was tryin to bench 225lbs and got trapped under the bar, I knew there was one other person there and I looked up and saw him in the mirror, a young mexican or honduran man was looking directly at me holding his backpack and keys. I managed to let out a "Help!" And a smirk came across his face and he said "adios blanco boy" and i heard the door slam shut and could hear a muffled "ja ja ja ja!" fade into the distance.
Lucky I was able to roll it down to my legs and get my breath or not sure what might have happened.

Assuming this isn't a troll post, just lift in a rack. Set the pins like 1/2" under your chest. Most people have a slight arch when they bench, if you fail just flatten out and the pins lightly catch the barbell. Push it up towards your neck and slide underneath it.
I never use a spotter but always use pins. Have failed countless times on bench press and never got into a bad situation using the pins.I'm 33 not 51
