Actually, walking at a light pace for someone of that size is more then enough exercise to burn fat. That is about all I'd have them do but if they are 500 pounds I'm sure they have talked to doctors before and probably already know medical conditions as well.
I'd probably try to find out how much they are eating a day. I'd have them log their food intake and see what kind of **** they put in their body. Depending on the results I would help them see what exactly they are doing and try to explain it.
Then I would set up a diet plan and make sure they drop a good deal of the bull**** they are eating. Their metabolism is probably ****ed up so I'd look into finding a healthy range first.
I'd reinforce the idea that it's not going to be an over night thing. If they really want to lose weight they are going to have to work for it and just like their fat didn't come over night neither will it be gone. I'd try to brace them for the road ahead the best I could and make sure they are ready for it mentally.
