Edit: I must be easily approachable because now that I think about it whenever I'm walking on the street people are asking me for directions and I'll always get picked out of a group by beggars or people handing out fliers and crap.

i would say this is probably what most miscers mean by girls approaching you.

What i would say...

Has a girl ever come up to me while i was not paying any attention to her and started hitting on me? no.

Has a girl ever put herself in close proximity to myself and all i had to do was turn to her and say hi? yes, all the time.

Has a girl casually interacted her way into my conversations and then proceeding to mutually make introductions? yes, all the time.

Girls are always going to be more timid about this stuff because they are just trying to fit their gender role; submissive. But if you aren't completely inept, girls will do a lot to show that they are interested in you and would like you to interact with them. The key is being able to recognize this stuff. For a girl, batting her eyes, making eye contact and smiling, or even as subtle as giving her body more attention to you (this can be as subtle as not crossing her legs in front of you or fully facing you, rather than kind of facing you at an angle), is pretty much the equivalent of a guy walking up to a girl he's never met before and saying hey i think you're cute as hell.

Yelling from a car is a lot different because they feel protected from the safety of their car and are likely just doing it to give their friends a laugh (notice a lone girl in her car will never "holla" at you, it's always cars with multiple females in it.
