Depends on the level of the competitor. In every sport, the top tier are going to be going to extremes to be the best, and it usually isn't being as healthy as possible. There was a quote/poll a year or two ago that basically showed that more than half of those polled of olympic level athletes would take a supplement/drug/magic pill etc that would win them a gold medal even if it meant they would only live another 5 years.
At the majority level, it is way healthier than your average person. A local/national level powerlifter or marathon runner is just going to be a stronger than average/ more fit than average amateur athlete. Squatting 500 or benching 350 at a bodyweight of 165-200 is not exactly putting the same strain on your system as a multiply 700 lb bench and 800 lb squat ya know?
