I'd say baldness. Nothing ruins a man's appearance and makes them look gheyer than a shaved horseshoe.

Having a full head of hair that's shaved like a buzzcut can look badass but a shaved horse shoe is about the most aesthetically displeasing thing a guy can have and will instantly turn a 9/10 guy into a 3/10.

It is pretty humorous how they shave it, as if we can't notice their horse shoe, lol.

I'm biased but I think MPB is way down on the list for one reason- it happens to men and only men.

There are things which are much worse IMO because at the end of the day they're not masculine:

Wide hips
Overly short
Gyno
Narrow shoulders

Those are just the first to come to mind.

Things that men find attractive in women, but if a woman sees those on a man it's not looking too good.

Same way if you saw a chick with narrow hips, flat chested, broad shoulders and really tall you wouldn't be attracted to her.

But on the other hand if a chick has cellulite it's not that big of a deal, as that's a 'woman's problem'.
