Guy took too big of dose. He then spent 50 years living as his grandma's couch. He kept trying to talk to his family, but he couldn't because he was a couch. He spent 50 years in real time waiting there as a couch until everyone died in front of him and they through the couch away. Then he came back to reality. It had actually only been 30 minutes in our reality.

I've heard a similar story of a man who got punched in the face. He fell into a 2 day coma but in his head, he lived 15+ years and got married, had kids and bought a house etc. It felt so real and vivid and genuinely like 15 years that he killed himself with depression from losing the life he lived.

It's basically this theme. Not an acid trip, but this same theme. It's one of the best Star Trek episodes and I think it's Patrick Stewart's personal favourite.
