This might sound a bit over the top, but.... if it's really that bad, you might want to consider using a bosu ball for sitting at work.
Or if that's too "girly," maybe wearing a lifting belt to remember to flex the abs/keep alignment could serve the same purpose.
And probably most useful of all, is to simply set a timer to go off every hour, stand up and stretch for even as little as 1-3 minutes.
Been trying for years to get rid of my back pain and honestly these were the only things to finally work for me :/
Oh and using a body pillow for sleeping helped also. I put a regular pillow under my stomach to fill in the gap between hips/lats, and then have a body pillow over to the side to put leg/arm over so everything is supported.
Edit: And one mental trick that I found helps with keeping proper posture for walking, is to think about walking more on your toes, like you would for jogging through a forested area. Pretty difficult to use unnatural posture when you're walking in a more natural state, it keeps the glutes & abs flexed so that everything lines up properly.

I'm at a new chiro now to try and I already like him better than my last. It's not like the previous one didn't help but the fact I get 15-20min with him instead of 2min means a lot to me. He actually gives me exercises/stretches to work on to help improve and said once you are corrected these exercises will help it stay there. It's only been 2 sessions so far. One thing he said was, come 2x a week for 3 weeks if you aren't 50% better or at least improving we need to try something else or get an MRI/X rays etc which I liked. It wasn't like the simpsons "Homer we need to see you every week for the rest of your life"
I found out my Ilium's (bone in ur hips/pelvis i guess?) are lobsided. My left one is a lot higher than my right and my L5 is sticking out and needs to be pushed back in.Workout Log

Damn, that sucks. Didn't realize it was an actual bone issue, just assumed it was more of a front-back muscular imbalance such as glute/hip flexor or abs/lower back.
Hopefully this new chiro works out.
So I guess you're staying away from DL/squat for a while then, ya? Always seems to be a constant battle with those for you.
