The bent trap bar in post #1 is something I haven't seen before.
But I also agree with you OP in that it probably occurred in the shipping process,
Hard to imagine a bend to the bar like that occurring while being used properly.[]---[] Equipment Crew #78 []---[]

Think this should be in here.
I had these chrome spin-lock adjustible dumbbells since I was a teen. They are still awesome and I use them all the time. However, the chroming starts to come off at this location. Now I usually use painted plates to sandwich the chrome plates in, but sometimes I bring the weight down , and they can be dangerous when spinning the lock. The chrome plating produces sharp jagged edges.
I still use them, but i just have to be cautious lolMy All American EFX Kre-Alkalyn EFX Log

I've got a cheap trap bar that's bent to crap too, not as bad as the one in the pictures but noticeably bent. Just from normal use, I think the most we ever loaded on it was about 500 so not even a lot of weight. Garbage.

You need a good rack, a bench, and a 300-lb Olympic weight set. Now, what was your question?

I use a 70s York every day and it's straight as an arrow. I'm not a crossfitter though. I looked through the comments and didn't see any details about the bar that she was using. It does appear to be a split sleeve. I wonder if it's a women's bar.
Here's a more recent photo of the same lady. It looks like she's switched to a fat bar.

You can get some rubber o-rings from the hardware store. They are usually in the aisle with plumbing/piping I believe. Just put that on the last plate before you screw it on. You can get a tighter rubber o - ring and put that on the bar (right before you put the first plate on). I used the #18 rubber o-rings. They can also be stretched to fit the olympic collars for a tighter fit. Works for me, and all the chrome is in-tact.
But if that doesn't work, you can sand it down I guess.

Apparently the "standard" isn't very high. ...is it missing the "D"?
