A few reasons:
1. His back. Dorian and Ronnie owned him during a time when a complete back was the key to winning the Mr. O.
2. His legs. They never really were up to par with his upper body.
3. Just being in the wrong place at the wrong time. There can only be one Mr. O at a time.

Too much time off between shows, injured back, torn pec which had to be re-attached.
Bodybuilding is all about image not numbers, when you're up on stage your lifting stats and bodyfat % means nothing to the judges.
