Mine are…
Calf raises/presses:
Literally no one does these right. No pause at the top, no paused stretch at the bottom to kill the stretch reflex of the Achilles’ tendon…..just throw on plates and do lil bouncy reps or worse, some kind of half ass power squat. Calf presses same deal, just bouncy reps.
Leg press:
Need I say why? We all seen them. The guys who throw on plate after plate to bend their knees a few inches. Who was range of motion?
Lateral raises:
At best, most guys turn this into a half ass high pull. At worst, people are flopping their arms up and down impinging their poor supraspinatus into the acromion. Either way, usually with entirely too much weight. 0 tension whatsoever on the target muscle because the traps and other muscles take over.
There are more of course but these are the 3 I see the most of. What about you guys?

Rows - 45 degree angle of the back, taking the bar from halfway up the knees to the waistline.
Saw a guy "row" 225 this way for a few strained reps and then watched him upload it to his snapchat with all of this bragging flare. Big eye-roller.Bench: 350
Squat: 405
Deadlift: 505

Rows- using a weight too heavy and looking like they're trying to hump something
Seated shoulder dumbbell press - only doing half reps because they're using too heavy a weight
Bench press - usually see young lads lifting weights too heavy with their arse off the bench while their mate helps them complete a rep.
Basically it all comes down to lifting weights too heavy for them.
I lift at home now but I've seen alot of funny stuff in the gym. On a separate note unrelated to this question, the funniest thing I've seen in the gym is a guy who had his mate spot him with 10kg dumbells on bench press while helping him complete his reps while he shouted "go on lad it's all you"
