A lot of people get baby clothes at a baby shower but they never come with clothes hangers. A few packs are cheap at Walmart and they'll appreciate it when they go to decorate their baby's room
Also cash is always appreciated, silver or gold coins in the year of the baby's birth if you want it to be more personal.

If you don't care about the people, get them a Walmart or Amazon giftcard so they can get toiletries and baby supplies.
If you care about the kid, go to the US Mint site and buy a current-year silver or golden eagle coin, depending on what you can afford, so they have some kind of memento w/birth year. If you're religious, buy a small silver or gold piece of scripture with their name/yr inscribed.
I buy entire rolls of silver eagles and give individual coins out for bdays. Write some kind of uplifiting note about setting goals and investing in themselves to watch their value grow, just like with precious metals. It's cheesy but people love it and the coins won't cost over $45 @ current melt value.Short, fat, and bald crew.

If they're first time parents or wife stays home, buy them some pocket (cloth) diapers. They're machine washable, the insert(s) that sits inside absorb the urine away from the baby's skin, so they won't cry as often for diaper changes, most importantly in the middle of the night. Less diaper rashes. And saves a ton of money in the long run, especially if it's a boy, who usually take longer to potty train. Get them a box of 6. They'll need more than that, but it's a start.

Good safe answer here. Diapers are always a winner. Maybe a sample of a few different brands to figure out what works best? Not all diapers are created equal, and some fit better on one kid than another. Clothes are great, too, especially if it's a first child. Formula (if needed) can be another helpful supply item. Maybe a changing pad for changing diapers when your sister is on the go.

I have 2 year old triplets. Diapers are always accepted. Buy a size up as well as that really helps.
