A solution to shoulder pain and knee pain

I broke my collar bone when I was a kid and recently I have been having shoulder pain in that area when bench pressing and shoulder pressing. I was wondering if there is anything I can do to resolve this without sacrificing my workout. I have also been having mild knee pain while doing squats and I don't want to cause any further injury. My form has not always been perfect but it seems like even with perfect form it is still a problem. I have been lifting for about 5 years and I want to make sure an injury doesn't get in the way of me continuing to do so. Any advice would be greatly appreciated.
