your hooks are like a hybrid between jabs and hooks.
if they are jabs try to full extend arm and POP IT
if its a hook, rotate torso/hip more with your jab like Tyson does
Also, I noticed your hands were low when punching - it can be a good habit to fire your arms up to protect your face when your throwing, when you keep your hands below your jaw you are open to punches - typically when you practice on heavy bag you should keep good form intact as well.
practice keeping your distance - like for example only come into range when your ready to throw the punch

Hit the heavy bag with a PLAN. IMO you're too free flowing/abstract on it. Leave that for shadow boxing. Work on certain combos/shots over and over again CORRECTLY (ie keeping your hands up), instead of doing all kinds of irrelevant stuff that's look like it's more suited towards conditioning. Laser focus on one thing and learn it right.
IE
1-2-3, hands up, slide out, or MOVE HEAD.
1-1-4, hands up, slide out, or MOVE HEAD.
