40 years of medical practice, never seen anything like this

I know misc doctors have so much more knowledge compared to the pulmonology director at 2:59, his creditentials and what he is seeing must be fake. Also they are trying to scare us with fake footage throughout the video.

It's odd that they lay them prone. The evidence suggests that it only works for a certain population of patients.

Italian doctors are, at best, only qualified enough to treat pasta and salami. If they were any good, you wouldn't see their patients having a mortality rate 4x higher than seen anywhere else in the world.

To say you've never seen anything like this doesn't mean it's the end all, just that it's different. I know two people who have had experiences in the Itialian medical system and neither one of them had anything positive to say about it. One, they were in the military but off base and got into a car accident. The other, she was on a tour and had a fall. Both said they basically went to the hospital, nobody knew what to do, they just layed there until they were able to get out and either get back to the base or back to the US.

Well you went through it, and you are in the medical field so you do know this stuff. The video said it is protocol for acute pnemonia? I guess your experience is that it only works for certain population.

I was being sarcastic by saying misc doctors, not talking about real misc doctors, only those that claim this is a regular flu/cold but have zero knowledge of the medical field.

Good luck this Friday, hope you are 100% by then.

American doctors that are seeing the corona patients are saying similar things like the italian doctor, does that convince you?

"I’ve never seen anything like this in my professional life.

Early on, from what we were hearing from the media, many of us were sort of thinking this is a disease really for older people. Last week, even one of my own colleagues was in the anti-panic mode, you know, trying to calm everyone down, posting that, “It’s not surprising when grandma gets a cold that she might die.”

But what I’m hearing from these groups now is that people are seeing folks in their thirties and forties getting severely ill. In the last 24 hours, just in these groups, I have heard of one paramedic and three ICU nurses that have been intubated. Clearly the denominator is huge, but even if it’s just a handful of critical cases of younger people, this is concerning and different from what we first thought.
    "

Those patients may have been in severe ARDS as can be seen with COVID-19. Prone positioning improves respiratory physiology (improved V/Q matching) and can reduce mortality in those cases.

It's a treatment used for patients in acute respiratory distress syndrome (ARDS). Basically, your lungs are failing and not getting better and you're on the verge of dying as a result. Very difficult to treat and reverse. It's an end stage complication of pneumonia, generally in populations with other comorbidities that effect their lungs and heart (smokers, COPD, etc). I've rarely seen it used on a normal bed -- a lot of facilities have special beds that rotate you prone, etc. but there's no way any hospital would have enough of those specialty beds. Most large facilities have 5 or less, and generally only 1.

I don't know if that hospital (or in Italy, for that matter) uses this treatment more often than other places/countries. It could speak to the severity of the lung damage being done to these patients. As far as substance goes I'd say that's what I gathered from it.

I was being sarcastic by saying misc doctors, not talking about real misc doctors, only those that claim this is a regular flu/cold but have zero knowledge of the medical field.

Good luck this Friday, hope you are 100% by then.
This is certainly a bit more than a flu/cold. More contagious than a flu (maybe due to vaccination?), but we don't yet know the extent of its pathophysiological intensity. It's just too soon. The early data suggests that it isn't much more than a flu of sorts, but without vaccination protection and it's probably airborne, which is probably why it's so much more contagious. Flu season always inundates hospitals with sick old people who end up on ventilators and in ICU, now this season it appears we have more than one virus causing that same problem -- increasing the amplitude. Spreading resources thinner.

Anyway, point taken on civilians 'knowing more' than 40 year pulmonologists.

Nah, man. You'd be surprised that your local hospital (such as mine) gets spread so thin during winter that a change like this could cause it to basically collapse.

In my facility during winter we routinely hold 20-50 patients in the ER that need to go upstairs for 1-3 days. Including critically ill. Increasing those numbers by even 10% would result in us doing CPR in the hallway and having patients who aren't critically ill turn critically ill because they're getting subpar care in the hallways or sitting on a barstool because we're out of chairs/beds.

Do you have an opinion on how we're going to be able to handle overflow? i.e. where is the bottleneck? Is it vents/equipment, manpower, physical space, drugs?

They have you think that this is all because of coronavirus. That's misleading because many of the deaths occurred through elderly patients who have underlying illnesses, health problems, and preexisting chronic conditions already.

Italy’s large elderly population poses a challenge in slowing the number of coronavirus deaths in the worst-affected country in Europe, a health specialist has said.

The virus has killed 79 people in Italy, overwhelmingly aged between 63 and 95 with underlying serious illnesses.

The youngest patient to die was 55 and suffering from chronic disease. A 61-year-old doctor who was not known to have underlying health problems has also died.

“Italy is a country of old people,” said Prof Massimo Galli, the director of infectious diseases at Sacco hospital in Milan. “The elderly with previous pathologies are notoriously numerous here. I think this could explain why we are seeing more serious cases of coronavirus here, which I repeat, in the vast majority of cases start mildly and cause few problems, especially in young people and certainly in children.
