This vid got me thinking decent squat stands like Rogue S2 / SML-2 with spotter arms are completely useless when you fall backwards. Is this kind of fail very rare? Even his unbolted rack moved a bit when he dropped the bar.

I disagree, and even might say the opposite. Being stuck in the rack, the bar would have folded him like a taco if he only had those bottom safeties and not the suspension ones. That could easily be a double hamstring pull. With an open/half rack, the bar simply falls onto the floor.

That's one of guys here who puts up amazing numbers (If I recall he was working on a different stance/foott placement when that happened) and if you look he has a rack, safeties and straps.
Beyond falling back, I'd be worried about falling forward, losing footing and anything else that could go wrong which is why I would never advocate using squat stands.
Like I said in my previous post about it, my view is Full rack >>Half rack>>>>>>>>>>>>Squat stands{5'7" and under #1}

I think it would have hurt a bit more if I didn't have the safeties And cage. With out the rack the bar would have been thrown against my mass storage.
The cage limited damage to my self and my equipment.Best Raw total 1850 at 181 lbs

I went backwards once. First time in a squat suit. ended up pinning my hand between the bar and the flipdown safeties. It took a trip to the ER and some stitches to get my finger fixed, and still dont have feeling in half my finger.
I dont squat with metal safeties anymore. I have straps hanging from the top like smokinhawk does.

Those were actually ring straps from rogue. I wouldn't recommend using them. After a few catches like that some of the seems ripped as it wasn't designed for that.
I have spud inc monolift safety straps now hung like that.
