Question RC tendon rubbing against bone

Hello how are you all? I have been having trouble with my left shoulder for the past 8 months, since going far too heavy on a lateral raise machine; my rear delts are also underdeveloped. I have been diagnosed with shoulder impingement although another physio put me through all the RC tests and says that it's fine.? X ray showed up normal as we'll.? I have been through 4 months of Physio and been prescribed more diclafenic sodium (50mg) 3 daily, more than I care to think about with NO success. The pain comes when I do Db/Bb presses or military presses. Especially when I put my left hand on my opposite shoulder then lift my elbow to my forehead. (Mazion test) It hurts my left shoulder. I am considering going for an operation to increase the space which the RC tendon runs through as this is what I have been told is the problem. It's so depressing being unable to workout my upper body as a result of a nagging f***** injury like this. Will this heal by itself in time or do you think the surgery would be a better option? Thanks in advance...😒

I'd suggest looking at surgery as a last resort. During your 4 months of PT, were you avoiding exercises that cause pain?

There may also be ways to work around the pain. On OHP/MP, are you moving the bar back over your head as you press up?

Are you able to do incline bench? That hammers the anterior delts pretty well.

It will not get better if you keep doing the exercises that aggravate it. You're just gonna have to cut those out of your routine until you get this sorted out.

At this point it's logical to have an MRA to see what's going on in there. An X-Ray will not show soft tissue damage, so an MRA should be the next step. Any doctor that would be willing to perform surgery on an injury like this without first ordering an MRA is worth steering clear of.

Go see a shoulder specialist and ask for an MRA. Then he will be able to determine if surgery is a good option for you.
