So I have used Gym Buddy since 2008, but the recent iOS update on my iPhone made it inoperable. It's no longer in the app store. I have since tried about 4 apps and most of them want to plan my workout for me and fail to even let me add custom exercises.
Can anyone recommend a good app that will simply let me track my sets/reps and also let me look at my history?

Gym Ace or Gym Ace Pro , pro is like 2 dollars but it is awesome. It will keep track of all your workouts and let you know what weight and reps you did last time on every exercise, also lets you add custom exercises.
I'm 25 , not 47 smh.

I use a Google Sheet spreadsheet. I typed in my list of exercises, warmup and working weights, number of sets, and number of reps for each exercise. In another tab I have a log of which workout I do each day (I'm on an A/B full body split) and what each of my working weights is so I can keep tabs on my progression when I get to the gym each day.

Notebook and pencil is king, but as much hassle to carry as a phone.
Use a spreadsheet, print the days workout on paper on half a sheet of letter/A4 and rip/ snip off the blank half so it is small. Then fold the workout in 4, and it fits in shorts pocket.
Remember to take it out of shorts before you wash them. Write any notes on it afterwards or update spreadsheet.

The phone plays the motivational music at 130+ BPM and is kept in my "old man hip sack"( wallet/car keys).

I use Gymbook it’s ok, I wanted one with a watch interface that was cheap. One day I’ll get round to coding my own, too many projects on the go to do it now.

I use one called Simple Workout Log not sure if it's on Apple I have Android. You can add exercises and it has a history and has a notes section. There may be better but does for me. Its free as well just a few unobtrusive adverts.

I've got one on my phone called Fitness Point Pro. It lets you export a workout via email or all of them via a csv file. It doesn't seem to let you create custom exercises though. I don't use it for logging so I can't vouch for that.
There will come a day when I tire of listening to 80's music. That day is not today.

Jefit is great. Been about 3 years since I did any logging, but it worked GREAT when I used it. They have changed their structure. Used to just buy the "pro" app and it was add free. Now seems they want monthly subscriptions (40$/yr) for the higher features.
Anyway...when I used it, it was awesome. Not sure if something is better out there now. They do have a free version that you could get the feel for it.

Was just coming in to post JeFit, I used this before I switched to Strong. It was very useable.
I only had a real issue with rest timers either not sounding when the phone went into 'sleep' mode or not being able disable 'sleep'. Strong gave me that feature and it was a winner.Training Log : https://forum.bodybuilding.com/showthread.php?t=175236371
