Can you include the link to the program? It doesn't look like it's active/clickable in the requirement list. This is really tempting for me, as I have a tendency to push myself way too hard to begin with--having a program and supplement that's actually designed to accommodate that might be an interesting experience.
Edit: I believe this is the study, but it seems to outline a template for a program rather than a complete program -- I'm wondering about the details.

All subjects participated in 12 week periodized resistance training consisting of full body workouts centered around the squat, bench press, and deadlift, and auxiliary exercises of pullups, military presses, bent over rows, barbell curls and extensions. Volume and intensity undulated such that Monday, Wednesday, and Friday subjects performed hypertrophy (3 sets of 8-12 RM loads and 60 seconds rest), power (3-5 sets of 1-5 repetitions, 40-60 % 1-RM loads, 2-3 minutes rest), and strength (3-5 sets of 1-5 RM loads, with 3-5 minutes rest) respectively for weeks 1-8. This was followed by 2 weeks of an overreaching, pure hypertrophy training on M-TH, and strength on Friday. The final two weeks, subjects tapered (50-80 % volume reduction) while focusing on strength and power. All subjects were placed on a diet consisting of 25 % protein, 50 % carbohydrates, and 25 % fat by a registered dietician who specialized in sport (RD, LDN, CISSN).

I would love to apply! I'm currently running a log, so it will depend on the date..
I meet all the requirements (over 18, live in the states, etc). And think I'd be a great candidate. I'm currently 5'10 174lbs (see attached pics), just cut down from 195-200ish and am currently lean bulking back up to the 200 area. I want to compete in my first competition next spring summer.

Why me?
I have logging experience and I've also beta tested various other supplements. I have an extremely hard work ethic and would do all that I can to make sure I do everything to the very best. I'm a routine guy, I almost have to stick to something so I could and would run the supplement/training/diet to a T. I've used various other LBM enhancers (X-Factor/X-Gels, Anabeta/ABE, Erase, Follidrone, and others I can't think of) to compare

My application.
Reasons & Accomplishments: I am actually a fan of muscletech supplement. Fell in love with the phase 8 protein chocolate flavored and been using it for a couple of months now and been getting great results. I feel like if I were chosen for this opportunity I would push myself beyond my limits and be very determined to achieve new strength and mass gains. I am also a very responsible individual who would provide feedback and post my logs as soon as possible as well as following the routine accordingly as to how it is. Being chosen for this opportunity would make me optimistic and give me strong determination. I am addicted to weight training now and I just can't stop thinking about it. What I hope to accomplish is just pushing myself beyond my limits in order to become a stronger and better version of myself. When I first started I was a 125 pound weakling who still had harsh memories of being picked on when I was younger. One day I went to the gym and was determined to change all of that. I gained a ton of mass and strength in a short amount of time. Benching a measly bar all the way up until my max now. I am determined to do whatever it takes.
Logs done in the past: None. Never had the actual opportunity to log anything.
Height/Weight: 5'9.5 178.8 pounds, 18& BF
Lift Maxes: Bench: 180/Squat: 245/ Deadlift: 265
Current routine: Babylove's Strength Program 3x5 program (The program you guys provided looks similar) Would be a great alternative to be taking and I will already have a good strength base to make more improvements while on the program provided.

Alright, I'm convinced--you've done an awesome job setting up the parameters of this log, and the idea of having a structured program coupled with competition in what ends up as almost a mini-study of its own really appeals to me.
Nyrin's Application
Stats:- Current weight: 168lbs @ 5'11" (still bulking)
- Current BF%: 17-18% (estimated; my abs are disappearing but I don't much care yet)
- Current lift maxes:
- Bench: 170lbs x 6 (finishing pec strain recovery at the moment)
- Push press: 135lbs x 6
- Squat: 225lbs x 4
- Deadlift: 245lbs x 6
- Routine: Modified GSLP (3x weekly full-body with the above exercises + some accessorization; 60-75 minute workouts + warmup)
- Diet: ~3000 tracked calories per day (+300-400 vs. TDEE), 200g+ protein, 70g+ fat, remaining macros discretionary
What do I want to accomplish? I only started my lifting a little over six months ago and, in that time, I've added almost 30 pounds of mass, starting as a 140lb weakling. I still have a long way to go to reach my long-term goals, but a big part of this is continuing to put on a lot more muscle. I'm extremely serious about my diet and training and always looking for good ways to accelerate and optimize my progress; BetaTOR and Clear Muscle look extremely promising for continuing to push me ahead in what I'm striving to achieve.
Why pick me? I'm a perfect fit for the needs of the HMB-FA use. Wow, so i went through all this information to see what this is all about and see what i would be getting into. I like the structure that is layed out in this program. Very strict guidelines and control which looks very appealing. I take my training very seriously and you guys have a very serious approach to this phase training. I follow a very structured training routine and am working very hard to transform myself to figure i want to be. I have invested a lot of time learning and training in the gym and have been working working a lot more on nutrition. Now that i have a good grasp on training and nutrition i am looking for little things that boost my results.
Current Weight: 267 (this actually inflated my scale broke and is reading approximately 9lbs heavy) and at roughly 22% BF
Current Training: 3 Day split, Chest/Back, Shoulders/Bis/Tris, Calves/Legs Maxes: BB@320, DL@455, DeepSquat@365
Current Diet: 2800 Calories, Staple items Chicken, Eggs, 90% lean ground beef, White Rice, Potatoes, potato bread and protien shakes
I take a MultiVitamin and creatine daily.
PreWorkouts are taken on an as needed basis and not neccessary
Goals: Transformation, strengthen and shape muscle while allowing body lean out.
Expectations: I am hoping for increased Recovery allowing to push my training harder and make bigger strides in strength gains

Why should I be selected? First and foremost I am dedicated to training and providing my 100% feedback and I ALWAYS keep the updates coming daily! I have a positive response to my logs and also have completed multiple logs for various companies including MuscleTech. After my stats I will include some of my "Sponsored Logs" for you to review. Currently this has come in a perfect time if I get selected. Not only will I have the time to dedicate the time and effort to this review but, the training schedule fits perfectly with what I am currently doing. HERE ARE MY STATS:
Weight: 176, 15% BF, MAX LIFTS: Bench: 280X1, Squat: 320X1, DL: 410X1, Current Routine: 6-Day Split: 2 PHASE (1): Monday: Legs, Tues: Chest/Tris, Wed:HIIT THURS: Back/Bi's FRI: Shoulders, Traps, Rear Delts & HIIT, Sat: Hams, Quads, Glutes (2) Phase consists of Power/Strength Building working on same muscles, but incorporating different sets/reps.
Diet/Nutrition: Have marcos figured out and currently use "Flexible Dieting." My macros are figured out for training days, Hiit Days, and Rest Days. Food pictures are always fun and I have included them in past logs with recipes!
What I plan on to accomplish with this log is throughly giving as much detail on what I experience and make an impact! Am I happy with what I have going on? YES! DO I want to take it to the next LEVEL? ABSOLUTELY! I have to...
I want to log this because I love seeing what different supplements can do and plan on bulking this summer as it is...simple as that. I know my mindset is there, nutrition is on point, motivation, focus...you name it. I'd just really like to see how this could add to the fire.

Routine: L/P/P, one accessory day, repeat; follow a Wendler 5/3/1 for my main lifts.
Diet: Right now I'm on a mini cut pre-bulk, macros at 55/250/200 F/C/P; plan on stopping Saturday the 26th and reverse dieting my way to maintenance and then to a small surplus. I am very meticulous with my nutrition and macro tracking.
