Ive had my form checked with light weight but never with the heavier stuff. I feel like it suffers when I start moving heavier weight so I want to see if you folks can offer any pointers or critiques. This is my last set 5x5 315lbs.

Other than your butt comming up just a bit before your shoulders, it looks good. Not gonna nit-pick; there'll be plenty of others here who'll do that.

Looks pretty good man. Lockout is a little weak, but I think that is because you are intentionally going slow to make sure your form is perfect. You could stand to speed it up a bit - both on the way up and on the way down. The motion used to drop the weight should be the same used to lift it off the floor. When you finish your knees and hips are locking out at almost the same time, but then on the negative you throw your hips back much sooner than you bend your knees - let them break at the same time.
Also as Will said - VERY nit-picky. If you changed nothing at all I think you would be fine.
