calling 40 to 59 for a nutrition survey

Hey all, I have a friend working on her RD (registered dietician degree) she has prepared a survey for her thesis and she needs to get 100 people, I'd appreciate if you can take the time (it's short 7-10 min) to fill it up
it's a anonymous and does not ask for any personal informations

Maybe change the wording of one of the last questions: "Do you agree that people must take dietary supplements?" -> "Do you agree or disagree with the following statement: People must take dietary supplements."

Not a big deal, but ATM I have enough time on my minds to nit-pick.

no questions just an analysis of supplement usage for this age group 40 to 59, the reason is there aren't many studies about this age group use of supplements so she picked it .. should be interesting
