Because they don't know anything about it. You can have a ton of money and go to someone for fillers or a facelift, when that wasn't even your issue. Or just get too many fillers without dissolving them, and they migrate, leading to a "bloated" look. They are normies with money. They just think, "oh no I'm getting old, better get some fillers". The only celebrity I know who did it right was Kevin Love. He got jaw surgery too. Paul Rudd has done well too (but he has primarily been aiming for anti-aging). Because he didn't go overboard.
I really dgaf about what you think about my jaw. I'll breath better and have a better sex/social/career/ life overall.

Some stuff is obvious. A lot of stuff takes lots of research into the anatomy of the face and various procedures. For example, nor wooding and a crooked nose is obvious. A poorly developed jaw or unaesthetic lower third is usually not (like Kevin Love's "before"). I think in a lot of scenarios, people just notice something is "off", without even knowing what. The brain can differentiate attractive vs unattractive in an instant.
