Amazing work OP, how much weight did you lose at the end ?
im 30..i stretch after every workout i do and it does wonders for me
stretching after leg training works for me.
well MRI will tell you whats wrong, so few days off will not hurt you....better that, then taking few months off because you ignored the pain and made it worse

Are you sure you are eating that much ?
Are you counting calories or just asuming the numbers?
Try programs like SS or 5/3/1 to push yourself and up those numbers

your form is actually pretty good, from this angle of the video i can't see that leaning you are taking about. For me it looks decent
add 1 more slice of that lettuce and you are good to go
i wouldnt be worried about biceps on this program...your main focus should be on the 3 lifts this program is based about, and your biceps will grow.
you can try pause reps on the 2nd bench day...did wonders for my strenght
