Lost Motivation
This is the 2nd time I've put on weight. The first time I kicked my ass back into gear but this time around I'm having trouble getting motivated. Plus I haven't been able to stay on track well with my diet and I need too. It's like I'm watching myself put on weight and not stopping it. Has anyone got stuck in a rut like this before? I'm really starting to see myself getting softer.

Take one small yet positive step you know you can do consistently.
Example, replace the rice you have at dinner with spinach. This will make it easier to make more changes.
Then create a definite goal, something you can write down in a few words.
Start small and build momentum slowly. Baby steps like in What About Bob?

Start with this. What is your goal? Is it to lose weight or to be healthy? Just remember that fitness is a marathon, not a sprint. This means you can reward yourself, just set a standard to make sure you don't fall off the wagon too hard.

Follow only motivation thread and watch body transformation posts everyday on imgur/ instagram, etc.
It works for me ;)

I don't wait for motivation. I take action and hope my motivation will catch up.
I command my muscles, they respond. Sitting around waiting isn't an option.
Pick something simple and do it. Endorse yourself for the effort. We get better at the things we do repeatedly. Good habits are built over time based on consistency.

Motivation is a lot like a stray cat - it comes and goes as it sees fit.
Just don't be an undisciplined lout, get up and do what needs to be done because its something you do, not because its something you [I]want[/I] to do. After a while, disciplined behavior becomes habitual behavior, and then you have made it.
Edit: Also like a stray cat - motivation will sometimes urinate on things you like before leaving.
