Pain through PT for disc bulge?

I know PT hurts when recovering from a muscle injury. But what about a disc injury (L5-S1 tore during deadlift)? I see back extensions are commonly prescribed PT for disc bulges but they seem to make the pain worse for me. Is that something I need to work through the pain for it to get better?
