Mystery of the middle abs

Question I’d like to hear your thoughts on. I have defined top abs and strong defined lower abs but the middle two appear completely missing cussing a dent shape side on. What the hell? Why is this happening and any thoughts on fixes to target this mid section?

Do you lift the same weight every set?

My main goal as a newbie is to build muscle, so progressive overload is key. If I can bench a weight for 6-8reps the first set, but I fail at 3-4reps the next set with the same weight, should I lower the weight so I can hit the 6-8 rep ranges or keep pressing the same weight even if I fail...
