Deadlift help required

So.... 5 years ago I put myself in an ambulance and had 2 days in hospital due to not deadlifting with good form. I took 6 months before I could do any exercise again... I totally ****ed myself up. Now, I have 1 yr of working out back under my belt and am considering deadlifting again....
