lets say you are in the gym or doctors office weighing yourself with one of those medical scales, and you have clothing such as a t-shirt, sweat pants, and shoes. should you subtract the weight of these items from what the scale says. If so, how much weight?

I'd say about 5-6 pounds if fully dressed

you have to understand that one's weight fluctuates a lot
If you drink a whole lot before you weigh yourself you can gain 5 pounds. The best thing to do is to weigh yourself in the morning on an empty stomach
