For example, a 4 oz of chicken on a package says it is 160 calories and 26 grams of protein. Does that mean that it is the serving size for a raw piece of meat?

Should be the raw, yes, as raw chicken contains more or less that much. Don't bother with the weight changes that happen from cooking, it's just the water content changing.

It should say raw or cooked. If that's chicken breast, 160 cals sounds like a cooked value.

BTW I just googled "calories in 4oz chicken breast cooked"

Generally has a little star next to the nutritional value on the back to tell you.

If it dosent check google/my fitness pal for a similar amount and should be pretty obvious.

Generally though I'd weight when it when there's no water weight. E.g chicken after cooked, rice before cooked.
