I have some 5+ year old shoe trees that no longer smell like cedar. Is it okay to renew them by sanding the surface of them?

I don't see why not; try a light sanding with 220-grit paper.

I buy new ones. They are only 20 bucks.

I sold shoes and cedar shoe trees and Nordstrom for years. We always recommended sanding. Just use a light sand paper and dust them off every few months or so. It'll open up the pores and allow the cedar wood to work its magic.
