Best to see a doctor or physio and get advice from them. Shoulders can be alot of trouble if not treated right the first time.
You could even have some nerve damage from the sounds of it. So rest and see a doc asap. See what he says.

Rule of thumb is only use ice during the first 72 hours for an acute injury.

For heat treatment you can get these packs that you press the button in them and it crystalisers and becomes very hot.
They work great. To reuse you just boil it in water for 5 minutes and then its reset and ready to go again.

Most shopping centres out here have them, especially leading up to xmas.

you don't use heat, because we want to slow down inflammation not add to it. inflammation is good it's how the tissue heals, but drawing too much blood in there at one time can cause problems. ice it and see a doctor or athletic trainer.

Always ask your doctor. They are professionals and will send you to a specialist if he can't help you out.

If it's been bothering you for 3 years you should atleast go get a MRI done so you will know the exact cause of the pain. If the pain is Rotator Cuff related, they take forever to heal because of linited blood supply. So heating the are for 20 - 30 mins. 2 times a day would be a good idea. Then maybe taking something for the inflammation.

Inflammation isn't good. I went to the docs for my shoulder and the first thing they gave me were anti-inflammatories. If the problem area is inflammed and irritated it will never heal. Sort of like if you kept picking a scab and never let the wound heal.

wrong, inflammation is a good thing. inflammation is the process of tissue repair. they gave you anti-inflammatories because our body tries to heal itsself too well. we want to slow down inflammation, but inflammation is THE PROCESS OF TISSUE REPAIR,which is a good thing

Inflammation of a joint is hardly a good thing.

Heat/Cold therapy has been used in sports medicine for years, however, as others here have indicated, let a trained professional determine your course of treatment. At best, you could be helping yourself or doing nothing at all; at worst, you could be doing more damage or prolong the healing process if you wing a treatment on your own.
You might get some good information HERE as well as to do some further research along with seeing a doc or the like.
