I usually pyramid my sets, taking them all to failure.
The final set of almost all exercises (except legs) I go at about 4 reps and that's absolute failure
Legs I just pump up my legs about once a month or so, really I train them pretty piss poor. It's funny though cause since I've done this both my calves and thighs are up an inch!!
