Dead bone in shoulder?

Has anyone had this shoulder issue? A friend of mine has 2 options to correct this issue... surgery or MORE therapy. He's torn. I was just wondering if anyone had this surgery or recovered well without it. All I know is the tip of bone in his shoulder is dead (????) and is causing impingement, thus pain. Nothing to to do with rotator cuff...

Any comments or passed experiences with this?
