Question Knee Pain from Running. Fix it?

My boyfriend and I have recently begun training for a marathon. We started this last year but had to quit due to knee pain. I went to the doctor and he said he was going to fit me for orthodics but never did. We just stopped running so the knee pain was no longer a problem. Now that we are back running it is starting to hurt again. I can go for a long time (hour+) on the elliptical at the gym and have not a single ounce of pain but as soon as we start running on the street or the track it starts to hurt. Here's the deal:

The pain is right in the front of my kneecap, slightly medial and it hurts directly underneath the kneecap. It hurts when I try to extend it, it hurts when I try to bend it, it just hurts. It hurt so badly when I got out of bed this morning that I could barely walk, and my boyfriend had to drive me to class. The more I walked on it throughout the day, the less the pain got, but it still hurts too much to ignore.

Some people have suggested that the muscles that hold my kneecap in place aren't as strong as they should be and that is allowing my kneecap to move out of place, causing grinding on the cartilage, etc. I was just wondering if anybody else had any knowledge about the issue and if they had any suggestions about how to treat/further prevent this from occuring.

    Hello!

First and foremost try not to run on roads, or at least don't train on them.
If you have to run on roads, run uphill. Running uphill builds the muscles in your feet and calfs and hipflexors, which all need to be strong for a correct less knee-damaging stride. And when you pick up your feet with each step (to run up the hill) your feet don't hit the ground as hard = less strain on your knees.

Try to run on trails. You'de be amazed how much give gravel or dirt can offer.

Train at shorter distances, for strength. Your legs get tired and your stride becomes lazy and more and more damaging to your knees (and back, feet, shins etc) If you're training for a marathon you're probably training primarily at long distances, which is great for cardio, but it won't build the muscle you need to have a correct stride. Try going for short, fast runs, and eat a good meal with lots of protien and carbs right afterwards.

Glucosamine Sulfate is a good join supplement to look into.

yes, sounds like you possibly have weakness in the vastus medials obliques muscle which can cause lateral sublaxion of the patella. This type of symptom causes pain in the back of the knee cap.

It also could be a cartilage issue from wear and tear from running and there is some sort of abnormaility....

It could also be something completly different that I didnt mention.

I would get a referal to an orthopedic specialist if I were you so they could give you a proper diagnosis and perform the proper tests to narrow the problem down.
