-mental illness
-social outcasts/bullying etc
-easy access to guns (before you start fuming, I am pro guns)
-lack of security and gun free zones
Basically there's a mentally disturbed kid, he easily gets a gun by buying it himself or stealing it from a family member who doesn't keep their guns locked up, then he waltzes into a school with zero security and zero guns and he is free to gun down all the sitting ducks.
We can try preventing this every step of the way
-try to get mentally ill people the help they need
-parents teaching their kids to stand up to bullies
-have proper psych evaluations and background checks before buying guns
-keep your guns locked up in a gun safe
-have armed security in schools (no they don't have to patrol around scaring everybody)
-get rid of gun free zones aka "sitting duck" zones

This might be asking a lot, but maybe it's worth mandating that all teachers, principals, coaches, anyone who deals with high school kids, be required to take some kind of mentorship training. A training that specifically teaches you how to deal with kids who you think may be outcast in a way that doesn't embarrass them but builds them up and gives them tools to become more socially adjusted.
^ The problem with that is that mentorship is hard to mandate. And the wider you spread a program like that, the more watered down it would be and nobody would take it seriously.
But lack of strong leaders and mentors in these kids' lives is probably huge.

Lots of reasons
> Idiots have access to guns.
> People are more socially awkward and it is applified even more by social media because they consistently see people having fun with friends where they're not invited.
> Killing is normalized in the US (people are fine with cops murdering people for petty crime, sometimes no crime. People are fine with murdering people to 'protect' themselves)
> so much media in general focuses on murders / killing / crime
> No good influences (rapper talking about killing, drugs etc)
> The traditional family values are gone and lots of single mothers
> kids see parents who have multiple partners which makes them think friends/love ones are disposable
> lack of social activities/everything is online with so kids get stuck in weird internet fantasy lives where it's a bunch of like minded individuals talking about crap with no consequences...
And the list goes on/

One other factor that merits being considered is rising social inequalities in the US, which I don't think were nearly as bad 50-60 years ago.
Many of these shooters are from poor socio-economic background. Often (but not always) White, a group that is seeing some of the highest decline amongst all ethnic groups in the US. SOURCE: http://jaybookman.blog.ajc.com/2015/...-now-revealed/

This book has a section on the psychology behind school shootings.
The author believes that violence is a mental health issue, but isn't widely recognized as such.
I highly, highly recommend this book. It's short, and incredibly insightful...
