The vast majority of people don't value athletes, actors and musicians as much as you think. Dropping $10 for an album, movie or $50 for a PPV is nothing for people. On the other hand, people would pay doctors $1000 without a single thought.

It's just a case of there being millions of doctors vs thousands of athletes/entertainers so the piece of the pie is distributed differently.
