Because I specifically recall reading about how Big Ramy has a unique advantage being from the Middle East (Egypt), where doctors over there can get ahold of uber-quality, uber-cutting edge stuff that's not even available (or as easily accessible) in the U.S. But even if that's true, I don't know how the logistics of that would work if Big Ramy in fact spends all his time in NYC or wherever he's at.
