He lost a couple of tourneys to Rhaegar but I'd probably still go with Dayne, Eddard stark was meant to be a monster back in the day and he said that Dayne was the best he ever saw. He's pretty much remembered as being the perfect knight. If you're talking living people then it's a harder question, Loras Tyrell is a monster but he's too small and Brienne of Tarth is better than him but she's mentally weak.
Maybe give Barristan a few black coffee's and he's the best, long as he doesnt break a hip.**#3 always delivers crew**

Barristan Selmy in his prime would of gave anyone a run for their money. He's been in more battles than most others and still lives to tell the tales.
Jamie Lannister was a great swordsman at tourneys, but how many real battles had he been in? No doubt he's good but I think he's more talk than anything. Likewise for Loras Tyrell.

Living? The vibe that I got from the book was that Barristan was the shizz. I'd think the list is like this;
Barristan Selmy (even old as fukk)
Jaime Lannister (before losing a hand)
Thoros of Myr (won the melee at the hands tournament with his flaming sword)
