    Question Private Gym Owners: what forms do you have trainers sign?

    When trainers first start at my gym, what forms should they sign? They will be independent contractors.

    When I am assessing a client for the first time I will have a client fill out the following:

    -Liability Waiver
    -PAR-Q (Physical Activity Readiness Questionnaire)
    -Client Info (Phone, address, occupation)
    -Operations Guide (expectations of trainer and client)


    Independent Contractors need to fill out the following-

    Employee Application (Trainer basic info)
    Proof of Liability Insurance
    Photo Copy of Driver's License
    Non-Compete Agreement (If they leave or get let go they cant take their clients)
    Operations Guide or Contract (What is expected from the trainer)
    W-9 (For Tax purposes this to get their tax I.D. number)
