Does anyone have a sample legal/health questionnaire for new clients they can share?

is there an online legal form (contract) i could use to help with liablility?

i always do various type of health screening and questions into prior/existing medical condition and/or injuries

but i figure it goes hand-in-hand with insurance

This was the case at one point, as liability waivers were filled with "legaleze" which made the documents difficult to understand, and they attempted to remove an rights a client may have. If those documents held up. Trainers would be able to do absolutely anything with no legal recourse if those documents were actually enforceable.

A liability waiver that is far and easy to understand / to the point should hold up. Here is one that I made, but I would suggest you look in to your state laws before deciding on a final document.
