is there an online legal form (contract) i could use to help with liablility?
i always do various type of health screening and questions into prior/existing medical condition and/or injuries
but i figure it goes hand-in-hand with insurance
(ive been told that it doesnt hold up in court but i always take full notes to document experiences)

A liability waiver that is far and easy to understand / to the point should hold up. Here is one that I made, but I would suggest you look in to your state laws before deciding on a final document.No refuge could save the hireling and slave. From the terror of flight, or the gloom of the grave.
