Legal Forms/Lifestyle Questionnaire etc Templates?

Hi everyone,

Has anyone got any copies/examples or templates of:

Legal forms/indemnity forms for the clients to sign (protecting the trainer legally)
Client Lifestyle Questionnaire
Client health screening form

Need versions of these for my company. Tried writing them all up but struggle a bit with layout. I have sample copies and examples from gyms ive worked at/my training booklets. but wondering if anyone has any basic templates or the like I could have and then edit accordingly? it would help me out no end.

someone must have copies they use at their gym? I don't want to steal anything from anyone, just need a bit of pointing in the right direction with a template.
