I think it should just be "SPF" or "SPF Fitness," even though that is redundant. Maybe make "F" stand for something else.

Your business name is the forefront of your brand, and I'm actually big on last name branding. Unless you have a totally unaesthetically appealing or absurdly common last name.
All of these ideas on the survey are pretty good though. But do you want "pretty good", or do you want a name that represents the entirety of you company vision in a professional and integral way? The end game is always that your brand becomes a household name - so just ask yourself, would you rather that household name be something clever like Knight & Day Fitness, or have the authority and distinction of being YOU? Your business should be a passionate extension of yourself, so if possible, go with the last name. It's a tried and true tradition.Rep 500+
