Your hips are rising first and locking out early. You never get your shoulders back and never really locked it out. Also, your lower back is rounding.
In general form check vids are better with something closer to a 5RM so that we can see your form on multiple reps, at a challenging weight that causes your form to break down, but not so heavy that you might injure yourself.My Journal (RIP 05/11 - 09/13):
