The head butt was an act of frustration from Ortiz as he simply could not land a clean hit. I can understand how some would feel Mayweather was disingenuous with his stance/posture (looking rather open as if the fight had not resumed) as Ortiz was apologising but if you go by the rules it's a legitimate KO.
