Ok, I had a bad form at one time as well. It seems that you need to put your foot stance a bit wider. Also Yeah, you need to stay upright, usually means a weakness in lower back, or you just havent been shown the right form. Be careful though, thats a great way to injure your lower back. As far as the height, once you get your form down better, it shouldnt be an issue. But it was abit high.Good luck

Something that hasn't been said, is that you never started in the correct position with your knees locked out. This really starts you off in a crap position, because your chest isn't up and your body is coming forward.
Please pull up your damn pants, or wear some underwear. No one wants to see plumber ass.

You need to start fixing stuff from the beginning, which is the set up.
Proper Set-Up Video:
Also, you need to learn to push your knees out on the descent and continue pushing them out on the way back up. This will help with depth.

I would suggest dropping the weight down to atleast 135, if not, the bar. And videotaping yourself again trying to have perfect form. Then you can see how much better or worse it is than your current video. Then you can slowly work the weight back up using current form. The extra practice will drill the good form in you until it becomes second nature.1372 @ 205

Oh and get that mirror out of there. You shouldn't be looking at a mirror when you squat. And those safety bars are there for a reason, use them.
