8-12 for size, but 4-7 for the strength that makes much more size possible.

    Depends on your goal.

    For 1RM strength; 1-5 reps

    For 10RM strength; 8-12 reps

    For 20RM strength; 15-25 reps

For size, work on your 10RM strength, using majority 8-12 reps

For Powerlifting, work on your 1RM strength, using majority 1-6 reps

I like to stick to a 6-20 range. If I had to narrow this down, it would be 6-10.
Bodybuilding is 60% training and 50% diet. Yes that adds up to 110%, because that's what you should be giving it. Change the inside, and the physique will follow.

Similar to me...I like having a variety in my workout, so I tend to have movements that go as low as 5 reps and some that go as high as 12...When I feel like concentrating on strength, I'll favor lower rep exercises, but still have at least 1 movement with 8-10 reps.

Failure is not a rep range. You can reach failure on the 3rd or 20th rep depends on the weight, but either way you have reached failure. Unless you mean some sort of drop set where reps ain't counted, just keep going down in weight til you can't lift anymore.

For the clean and jerk I mostly work with singles, with rear delt raises I go up to 10.

All depends on the exercise and goal.
    Strength + Speed = Power

If you never fail, you aren't truly pushing yourself to the limit. If you never push yourself to the limit, how do you know what you're truly capable of?

Training to failure isn't always the best idea and doesn't make one a pussy if they don't train to failure.

Have fun training to failure on every set. I'm sure your CNS will love you for that.

Failure also isn't the optimal method for muscle growth which was the initial question. Don't jump to conclusions.
