    Bulging disc will not heal - Any help at all appreciated

    Cliffs:

    1. How did you fix your bulging disc / learn to live with it?
    2. Anyone know any top notch specialists in the Denver, CO area?

    Full version:

    I've been dealing with a bulging disc in lumbar region and sciatica for about 8 months now. Did some research on the forums and found a few old threads but not a whole lot recent. I've been seeing a physical therapist but nothing has helped.

    1. Dr. tried oral steroids first
    2. Physical therapy ever since

    I have not tried injections yet. I hear those are just a temporary relief and do not address the underlying problem.

    It feels severe, but MRI shows that this really isn't a severe bulging disc. A spine doctor told me that he wasn't ready to do surgery yet but that if we did do surgery, it would be pretty minor. I'm really starting to lean in that direction.

    If anyone has any experience with a bulging disc, can you tell me what you did to get better? This is really starting to become depressing. It's affecting gym, work, social life, everything. Cannot extend my right leg and get tingling sensations on top of foot on a regular basis. I told two doctors about this: general practitioner was very concerned, spine specialist was not too worried.

    A few people have told me that if conservative treatment doesn't bring relief within 8-16 weeks, it's time for surgery. It's been 8 months now so I'm definitely beyond that timeline.

    However, I'm willing to give a good sports medicine physical therapist one last try. I feel like I didn't get very much individualized attention at my last PT and may have missed out on actual treatment.

    Also - will I ever get back to deads and squats? I have a minor curvature in my spine which probably doesn't help. Is it a bad idea to put heavy loads on a curved back? Doesn't everyone have a curved spine to some degree? Either way, I'm willing to get creative with leg and back lifts, but I will not ever completely stop lifting no matter what. It's too much a good part of my life to ever quit.

    Anyways, sorry for the novel. Anything you have to say at all is much appreciated. This sucks more than I can explain in words.

    I'm starting to think I'm in the same boat as you. Initially I was told my injury was most likely muscular and to be fair that was probably a reasonable diagnosis given that I had decent ROM and little pain (this was 5 weeks after injury). Now my case was slightly different because I actually did not feel anything at the gym that signified that I had injured myself. I woke up the next morning with stiffness and then started having sciatica.

    Anyway, after 3 months I'm still getting radiating pain in my thigh and butt when I've been sitting for a few hours. Nothing when standing/walking and the pain is very mild but I expect the original injury was disc related and actually my sports ortho said it was possible it was a minor bulge (I only got an xray so it wasn't definitive).

    What I've done in the past 3 months is rested a lot (first 2 months did not work out at all) and spent 20 or so minutes every day in my hot tub.

    Then I started back at the gym with about half of my weight on all exercises and dropped some exercises completely (DB/BB rows, military press, squats, lunges, deadlifts).

    Basically I'm avoiding overhead presses and anything that puts downward pressure on my back.

    8 months is a long time but I don't see why you shouldn't try another dose of PT before going the surgery route.

    As for whether you'll get back to deads or squats: no doubt about it. The question is what kind of weight you're going to be able to push on them and how quickly you'll be able to move up in weight. I expect if you do get surgery then you'll be in a much better position to get back faster.

    Bottom line is bulges do re-seat themselves and will heal over time. As you probably know some people have them and never even feel them.

    When the injury first happened I thought I had herniated a disc or something and I was adamant about not being out of the gym for long. After just a few weeks it became clear I had no choice and it was depressing the hell out of me but it's always good to remember that you have basically the rest of your life left to exercise so in the grand scheme this is not a big set-back. It's also important to remember that you are dealing with a pretty sensitive part of the body and you do not want to be too stubborn lest you end up with an even worse injury. There were some days during my recovery that I said "f this, I'm going to work out. I don't have time for this bullsh*t injury. If my back doesn't like it, tough." Then I'd sit and think and realize that the back is not a conscious entity and doesn't care about inconveniencing your work outs and is not trying it's hardest to stay strong for you. If it gets pushed past its threshold it WILL fold like a lawn chair and it will cause you a ton of pain and even more time out of the gym, so best to go about things the right way.

    That was a great post to read, IK. Thanks for taking the time to write that. You definitely sound like you've been there and done that. It's really good to hear something positive from someone else. I have been trying to keep my head up, but it can just wear you down after constantly being a pain in the ass (literally).

    I'm right with you on the sitting down part as well. I work in a computer chair all day and that kills me. I feel amazing when I get up and do some cardio on the elliptical though. Running is out of the question for now, but morning cardio is the best thing in the world. I'm going to be looking into a part time job just to get up and around more, even if the pay is bad. Wouldn't hurt to meet some new people anyways.

    I too avoided all overhead presses, squats and deads at first. I've been working back into them lately, but I think it's too early. Going to back off those again. For shoulder presses, I use a press machine that sort of leans back at an angle so the pressure isn't pushing directly down on my body. Working legs has been tough. Just been doing bodyweight squats, curls and calfs. Tried single leg dumbbell squats for a while but that was too awkward and asking for even more trouble. I am going to look into maybe holding a kettleball just to add a little difficulty to the bodyweight squats.

    I'm also with you on the feelings of telling your back to suck it up and quit being a pussy. I get so frustrated I feel like "teaching it a lesson" or something. Sounds stupid but that's how annoying it is.

    Wish the best for you too. Hang tight and get through this BS. You definitely have the right mindset to survive this temporary setback.

    I hurt my lower back in November and thought I herniated a disc as well, a doctor and a chiro basically said it could be an inflamed/bulging disc but likely not herniated because I feel no pain anywhere other than directly in the center of my lower back.

    X-rays show that the space between my L4 and L5 vertebrae (I think that's what they were) is smaller than the rest of mine, so I assume that disc has been bulging or smaller for a long time.

    I've been trying a lot of stuff from trigger point therapy, stretching, and even going back to the gym and doing some lunges, hyper-extensions, etc. Some days feel fine, others I feel sore.


    I bought this book this week though as I hear very good things and I am going to read it front to back pretty soon. You might want to check it out:

    Damn you guys are great. If you've been where I am right now, it's awesome to hear other people chime in and share. I really do appreciate it.

    Yup, first thing I'm going to do this week is find an amazing physical therapist. I'll be looking into someone who specializes in spinal work or at least sports rehab for actual athletes. My last therapist seemed pretty good, but he had an intern college student do half the work, then my PT would needle me, then set me up on the traction machine and send me home. There was very, very little exercise in the office. I'm going to look for someone who might actually have me bust my ass a little bit in the office.

    Different people seem to respond differently. My back has never been good (runs in the family) and I aggravated it pretty badly last spring. MRI revealed a herniated L5-S1 with nerve root impingement, facet joint arthropathy and some scar tissue build up. Through visits to the chiropractor/physical therapist for stretching, that stim crap, adjustment and massage plus stretching on my own I got back to squatting heavy. I aggravated it badly again in December. Heavy squats + attempt at deadlifts (I had been avoiding them since the spring) + sitting in a bad car seat for 11-12 hours the next day = it hurt like a biotch and I could barely move. I could literally barely get up off the couch and walk to the other side of the room, and that was ugly, painful and difficult.

    Overall as long as I avoid deadlifts (something about them...) it does well. Good form squats seem to help. My biggest issue was always range of motion. For the longest time I could barely bend over and reach to or past my knees. I can now bend over and touch the floor anytime during the day I want. I credit the increased flexibility from the stretching and proper full ROM squats for the overall improvement.

    When I first went to the chiro/PT last spring the assistant PT said I was one of the most "tight" individuals he had ever stretched. My ROM was absolutely ridiculously terrible and eventually, after several weeks of 2x per week, he was able to actually get a decent stretch in. REAL stretching is not enjoyable...but it is worth it.

    I was in severe pain for about a year.

    I stretch everyday and i swear by it. the only thing that ever helped.
    Have you tried chiro? because of the disc my m uscles got tight and causes my spine/hips out of place. putting those back into place help immesnsely.

    The only thing PT did is tell me good stretches to use. Massauage therpaty helped big time.

    try chirp/ try massuage. Stretch everyday man.

    I do flexibility and stretch training. sorry im drunk but. try ALL options before surgery.

    It's hilarious how you talked about "teaching [your] back a lesson." I went through the SAME thing. I'd get so mad; rant to my gf about how I gave my back 2 months off and if that wasn't long enough then tough sh*t because I couldn't be out of the gym any longer. I wished that my back was a person so I could kick his ass for being such a puss.

    Over time you reconcile your frustrations. I began thinking that if my back was conscious and could talk it'd say "dude, you think I enjoy this? I want to be back to normal as well and I can't do it without you. Help me help you!" In conjunction with my gf telling me to stop being an immature, stubborn douche this changed my thinking about the whole thing and likely prevented me from doing something dumb (i.e. ignoring the injury and going back to the gym too early).

    I'm really not sure how I injured it. I've always had a sketchy back, but never any major problems. The symptoms just slowly crept up on me at the end of last summer. When I was doing normal leg stretches, I started noticing that I couldn't stretch my right leg because of some kind of shooting pain in the back of my knee. At first I thought it was something to do with my knee or hamstring. Things just got worse from there.

    My guess is it could have been a combination of heavy squats, deadlifts, sitting in front of a computer all day and just having a ****ty back to begin with.

    Has anyone assessed the irritability of your neural connective tissue?? and/or performed neural glides?

    Hey guys, sorry if I repeat some things already said. I just skimmed through all the replies. I'll try to make a long story short. My back was so bad from my sport that twice I woke up in the middle of the night and my back locked up and I couldn't move AT ALL. If the house was on fire, I still couldn't move. With that being said, my back is/was pretty bad. A good chiropractor will fix you. You have to make sure it's a good one. I've relocated a few times and unfortunately I had to go through a few before I found a good one. Therapy will use heat/massage/etc. to relax your spasmed muscles so your spine goes back where it belongs. A good Chiro will gently put your spine back where it belongs so your muscles don't need to be in spasm. It's just a much quicker way to acheive the same goal. Put ice on the area and take ibuprofen to start working on getting the swelling down (swelling is creating some pinching of the nerve too so get swelling down). Get an inversion table. You don't have to go all the way upside down to get results. Just google inversion tables and everything that is says is true to the word(even if it's a page trying to sell their product). I've had mine for 10 years. It's worked miricles for me and I've had friends that exhausted everything and were about to have surgery and they tried my inversion table and now they don't need surgery(they eventually purchased their own and continue to hang a few times a week). If all else fails, find a chiropractor that is certified to perform "spinal aid". Again, google it and what it says is true to the word. Inversion and spinal aid creates more space inbetween your vertebra so the discs have less pressure on them. It also replaces the fluid in your discs so you have more of a cushsion and healthier discs. If you move during inversion or if you do spinal aid, it will create a vaccum to suck the discs back in to where it belongs.

    This is the therapy I use as well as many other friends that I've shared this info with who are now living painfree lives. As far as different exercises to stay away from...squats, lying leg curls, deadlifts(unless very light). Leg press is fine if your back is supported, just don't go so heavy that your back "rounds" as you push. You can do seated leg curls, just no lying leg curls.

    After you get feeling better, google "swiss ball exercises". Over a few months, these will really stregthen your supporting back muscles because it puts them in balance movements that makes your brain more aware of these stablizier muscles. And stretch, stretch, stretch your hamstrings and glutes.

    Good luck everyone, I hope at least 1 of the people who read this follow my experience and I thank the ones who were before me who shared their experience with me so I could be painfree again.

    Awesome thread guys. Very informative.
    I'm currently sitting at work with a big ice pack attached to my lower back cuz I messed it up (again!) yesterday doing deadlifts. I did it first a couple of weeks ago deadlifting and thought I was all healed up. Wrong! Now I can barely stand up straight. The pain is pretty bad.

    Going to the doctor later today and hopefully will go see a back specialist or at least get some tests done. I've had back issues here and there and I think there's something wrong.

    Just when I was feeling like was making some good progress, this happens. I'm pissed off and depressed. And the thought of not being able to deadlift (if it comes to that) is not pleasant.

    Try not to be depressed. 90% of back pain doesn't need anything more than the right therapy, a few weeks, and learning what not to do to aggrevate your injury in the future. Herniated or bulging discs feel like major, crippling injuries with no hope. While the pain is VERY real while your discs are acting up, it's not a death sentence for life. I can't stress my earlier post enough of the way to be painfree. I agree with seeing a doctor, but it must be a chiropractor. They are spine specialist. Medical doctors will just give you pills and tell you to rest. Maybe send you to therapy to get your muscles to get out of spasm so your spine can go back to its original position and so the discs can go back to where they belong and stop pinching on nerves. A chiro will put your spine back and that will make your muscles go out of spasm. Its much, much quicker. If you can work chiro and PT in together, that's even better. But if you just pick one, it must be a chiropractor. Remember that the pain you're feeling isn't from the torn or bulging disc itself, its from the nerves being compressed from the disc bulge or disc fluid in the space where the nerves are.

    This guy is speaking the truth. I've heard about so many miracle fixes. Fixing or rehabbing a bad back is different for everyone. See a doctor, PT, orthopedic doctor before you do anything crazy. And be very wary of chiros. I'm sure there are some good ones.. but they aren't always up to par. Gotta be careful when you have major nerves being pinched.

    Lately I've been focusing a lot of effort on stretching everything in my hip and groin area to loosen things up in that area. I've read that muscle problems in the hip/pelvic area can cause the canal for the sciatic nerve to become narrow. I'm not sure if this is my problem or not, but the stretching seems to be helping.

    No, never had that assessment. But nerve glides were some of the main exercises my last PT sent home with me. I didn't notice any major changes doing them. It might have helped a little.. but I'm not sure.

    have any of you guys used any supplements to help you recover like glucosamine, celadrin, hyaluronic acid? im gonna try this stuff called alflutop that supposedly works good. i was thinking about prolotherapy too.



    i hurt the right side of my lower back about 2 years ago and it would bother me on and off, now i cant even play basketball. i seen a chiropractor last week and he wasnt very informative but i could see on the x ray that something in my lower back was pressing against my hip bone causing a clicking noise and the disc was more flat than the other ones. when i see him i lay on my stomach on this thing and he moves my legs around twisting my lower back then pops my back and thats it, takes a total of 3-4 minutes then they attach those pads to my back for 8 minutes. but the clicking isnt as bad now and im think about seeing a PT.

    I would be dubious as to glucosamine and celadrin speeding up recovery to any significant degree. In other words, I think you'd recover pretty much just as fast without them.

    Alflutop, however, is a real drug and I've read many good things about it. Go see an orthopedic, by the way. Not sure why you're wasting your time with a chiro. You have real back problems, not just some misalignment.

The know the spine, spinal nerves, discs, etc., better than any other kind of doctor. In case it was missed, I'll say it again...Chiro's are doctors that specialize in your back. They study and major in only your back. But what do I know, maybe people with back injuries should see a shoulder doctor or eye doctor or a dentist...ugh

    IMO a "real" Doctor has an MD.

    For example, at the chiropractor I went to the chiro was just that...a chiro. The Doctor was an MD who specializes in sports medicine. He was the doctor there...not the chiro. In fact, when they ordered an MRI the chiro said he would have the MD order it because they (insurance mostly) wanted a "real doctor" to do so. (paraphrased)

    Keep trusting the opinion of an insurance company to oversee your health instead of a doctor that spent 6 years taking the same medical courses that a surgeon took, but in addition, took specialized education just for back, spine, discs, etc. to become a back specialist.

    I would trust an Orthopedic Surgeon before a chiro. I'm not bashing on all chiros by any means. I just think a little extra caution should be taken with chiros.

    However, with an orthopedic surgeon, you have to make sure you get someone who isn't going to throw you under the knife the first chance he gets. I've learned that no matter who you see, it's still a human capable of screwing up. Get referrals, ask around and trust your instincts.

    Chiropractic can be helpful with relief of minor back pain. Any further claims about its efficacy should be ignored.

    Your point is bullsh*t anyway. Who said someone with back pain should see an optometrist? People with back issues can find orthopedic back specialists and those people know not only as much as any chiropractic doctor about the back but a lot more about how to treat the related problems. In fact, if you were ever to go see a general orthopedic with back problems he would undoubtedly refer you to a back specialist.

    Chiropractic was born from the concept of vertebral subluxation causing all disease, started before modern medicinal knowledge came about, and there has never been anything to suggest it is significantly more effective than taking NSAIDs, stretching, and resting.

    Many chiropractic doctors know this and practice their profession within its limits. Many do not.

    If you have some mild soreness in your back you might consult a chiro. Anything else should be looked at by an orthopedic back specialist.

    And to claim that choosing an ortho is choosing an insurance company to oversee your health while choosing a chiro is choosing a real back doctor is laughable and idiotic.

    Well I woke up today feeling the best I've felt since I hurt my lower back in November.

    Went to the gym, and picked up a 10 pound dumbbell the wrong way and BAM .. immediate pain. Been on and off icing it since I got home and took some Aleve.


    Crazy just how something little can re-aggravate it again. I guess I really wasn't even close to being "healed" and my bulging disc was just beginning to die down. Hopefully this isn't too much of a set-back but it definitely hurts when I bend over even a little bit.

    Damn dude, that sucks. Hope it's a minor flare up.

    I'm a month in to my return to the gym (finally back to the weights I was doing when I was injured in december) and I poke and prod my back after each set on each exercise. Sometimes I feel some mild radiating pain in my thigh for 5-10 seconds.

    I also get inflammation in my my mid-lower left back (I think it's a joint) after pretty much each workout. It's gone within 30 mins of ending the workout. However, both of these things scare me slightly because when I re-tweaked my injury back in February I didn't feel pain during the workout. I felt pretty normal just like I do these days. It wasn't until the next morning that I stood up and got a shooting pain down my leg.

    So the following morning is the big test for me and so far I have woken up every morning feeling good with no pain.

    Finally started incorporating some overhead presses again on cable and DB at 40-50lb (total) and so far so good.

    Starting my off-day cardio training again tomorrow on bike and maybe elliptical depending on how it feels.

    Even squats and deads are in the horizon for me, although I'm going to wait until I have absolutely zero radiating pain.

    Still surprised how long this is taking to heal. Injury was December 16th so almost 4 months and still mild sciatic pain and I'm pretty convinced at this point my injury was not disc related, but I guess a problematic joint is almost as bad as a bulging disc.

    My back is a little wonky today. Hopefully it is better tomorrow, not worse. I carried tile and mortar and crap from the store to the car and then into the garage, followed by some work tearing up the kitchen floor. Did something it didn't like.

    Yeah man lifting and carrying sh*t for anything longer than a few seconds is something my back is not fond of either. I recently had to hire moving people to move some fairly light stuff into a truck for me which pissed me off and hurt my pride because if I was totally healthy I could have easily done it myself or with the help of a friend for free. I'm just not willing to take the chance anymore. I would punch myself in the face if I tried to be a tough guy and re-tweaked the injury. One re-tweak is plenty for me and had me out of the gym for an extra 2 weeks. No more.

    The "good" thing (I think) about my injury is it's localized to my lower back. I've never gotten pain anywhere else in the body like legs or flutes because of it -- so no sciatica.

    Woke up today and still feel in pain. Icing it again now, then I'll switch to heat tonight or tomorrow.


    I was doing lunges and Bulgarian split Squats yesterday. Felt great! And really tested my core and stability - but maybe it was just too soon.

    I'll just stick to upper body workouts that don't really use my lower back and continue my lower back rehab exercises only (Stuart McGill).

    A couple points, I skimmed a lot of this but have extensive experience from the insurance side of things -

    1. Recovery/success rates for surgery and non-surgery paths are statistically the same. So unless it's absolutely necessary, don't take the surgery route. (In point of fact, the negative outcomes with surgery are severe, while the negative outcomes of non-surgery are pretty much where you are standing right now).

    2. Physical therapists are trained to do chiropractic manipulation. I didn't see that mentioned and people are jumping on going to a chiro, but you are limiting your recovery paths by going to one. They do not have the clout nor the resources of a skilled physical therapist.


    As was mentioned above at some point, your best bet is a sports focused spine surgeon that works on athletes. Find out where your local sports team goes (for example, in northern Illinois I talked frequently with the Dr. that works on all knee cases for the Blackhawks). By doing this, you get the best range of treatment options, especially when insurance gets involved. Having a strong primary MD allows more referrals to sub-specialists and you have a better chance of having more stuff covered (or rather, less denied).

    Well, I was wrong about not feeling pain today anywhere other than my back.

    While I was lying on my bed I randomly lifting my legs while straight and while my right leg didn't do anything, I felt some tightness/numbness and radiating pain when I lifted my left leg -- obviously sciatica.

    ****.

    Going to see a chiropractor tomorrow to check up now as this may be worse than I originally thought. Guess I don't just have a bulging disc now -- but possibly it's become herniated.
    I've still got a lot to learn.
