I use near perfect form on the flat bench. I contract my shoulders and glutes, arch the back and keep my elbows in. Do I do the same for the incline? I injured my shoulder last time not doing this, and I don't know if it was form or if it was bring the bar to my collar bone.

Sounds like you have good functional form on the BB Flat Bench. Inclines should be no different. If concerned, lower the weight. Focus on form & technique. If shoulder issues persist, try using DB's instead. With regards to bringing the bar to your collar bone, I would advise against it. Too much torque is applied to the shoulder complex once your arms break plane. Upper arms should be parallel with the floor. Any BB Bench press is a high risk excercise in terms of shoulder injuries.
