Wow, that's terrible. This is why you should always use a power rack when you're working out by yourself.

Tragic.
However, it probably could have been avoided.
If you lift alone, You should never use collars.
But those who fight for right must remember St. Augustine's sage words,

Yep, been there ^^^ But I'm thinking he probably dropped it on his neck. You could probably lay there for quite a while with 84k on your chest, it certainly wouldn't kill you, and I'm sure you'd figure out how to get it off eventually. If you tore up your shoulder mid-set and the bar comes crashing down on your throat... you're done.

i felt so bad hes my age and the same weight as me.. if any1 ever ask me to spot them i well always this great hobby should never come to this im gonan invest in a power rack as i work at home.
wow. How do you manage to die from that? you could easily roll the bar down to your thighs and survive that way, yea it would hurt but its better than dying. And if he wasnt using clips he could of easily tilted the bar from side to side and let the weight slide off.

Yeah, if you get to the bottom and can't get it back up, I agree. However, if he dropped it from a height above him, that could have a far worse and deadly impact.
Based on what I'm reading, including his size, age, and the amount of weight, it is almost certain he dropped the weight on his neck or collar from some height. Otherwise, he could have saved himself, even if he had collars on the bar.
(It is also possible, although unlikely, that he had a heart attack.)
However he did it, it is a tragedy for the family.

He probably was using the "Suicide Grip" where you don't hook your thumbs on the bar.
You can guess why they call it the suicide grip.
Years ago, when my sons were babies, I trained much more at home than I do now.
I bought a cambered bar (standard, not Olympic) like this
and two racks to put under the ends of the bar.
This allowed for a full range of motion but would stop the weights if I failed w/o crushing me.

That was such a sad story...there was a young guy at my old gym in johannesburg who dropped the full weight of a heavily loaded bar right on his nose!!! he was in extremely bad shape and needed extensive reconstructive surgery. another time my 11 year old son was testing out my barbell and i just heard him yelping 'HELP HELP' from upstairs, he was trapped on the bench with the bar over his chest! (he was fine btw - just a bruised ego..) Personally I wont go near a loaded bar without assistance!
Sad story. Those of you saying a heart attack may have happened, but unlikely, I would have to agree. A buddy of mine who works out at my gym had a heart-attack mid-lift doing over 315 on the bench. He's ok now. The doctor kept him out of the gym for several months following that incident.
