I've blown out a couple pairs of pants and had to leave mid workout. This has happened to me twice.
My brother was doing DB bench, and one of the DB's came apart and he fell off the other side of the bench. . .it was funny as ****.
Last but not least my old workout partner mis racked a heavy set of squats and that whole side fell down on the safety pins. That guy was crazy and every time he squatted I thought there was a good chance someone could get hurt. . .he was a great workout partner though.Training Journal:

lol i was on the treadmil and usually i press stop and just slide off the machine with whatever power is left after i press stop. well i did that as usual and a girl was walking right behing me at the same time and i could stop at that point and i hit her and we both his the floor.
My mom thinks i'm cool

was DB benching 105's in each hand. I busted out 5 and thought I could get 6, I got the 6th rep up with a spot, but my arms were so tired that my grip loosened and I dropped the DB's
one bounced off my chest and almost bounced into my face and the other blasted me in my hip bone, felt weird man. I had a big ass red scratch mark on my hip. my chest was fine. My spotter freaked out lolSTEAM: rangetsu1 | SC: jonnyboiiir | IG: jonnyreyes12

i embarrassed myself and it just happened the other day. i was resting during benching and all of a sudden i scope out this fine ass "girl" with a dat ass. "she" is far distance away and standing wither back towards me. all of sudden "she" turns around and i realize its just a guy with a nice ass. i was really embarrassed towards myself.

I was feeling good one day, feeling strong n ****. Gym was packed with noobies and ladies, so i was trying to show off a bit. Was doing 2 and a half plates squats 8 reps for warmup. I was thinking F this I can do 3 plates perfect form too no problem.
Well didn't go as well as planned, i got low with the weights like is usually did, legs at 90 degrees, I start pushing up, and my lower back gives out, causing the weight to go forward, with my neck under it. Got really lucky as the squat racks have this lower level, and it basically stopped the weight from falling on my neck lol. Made huge noise, whole gym looked over....... first thing i checked is if the cardio bunnies were looking over and laughing, but thank god they werent.

Mine's decent
I usually use the gym at work...I was doing plyometrics after a leg workout, jumping onto three stacked Reebok Steps, probable 30". They get kinda wobbly sometimes, but usually no problem if you are controlled. Well this day I got real tired after a few sets, and jumped up, landed weird, knocked the stack over and fell on my a$$. I was fine, everyone there were co-workers.
After that, people were busting on me about it weren't even there that day.

I was doing over head press, bent down for my water bottle, came up and smashed my head on the bar.
Was adjusting the incline on the bench with one hand, lost grip some how, and the bench smashed down on my thumb, still recovering almost a year later (still sore sometimes).
Went to grab a 35 lb plate from the second to bottom ring, and knocked two 45 lb plates on my foot.
I have ****ed up more times at the gym than anyone. Finally getting results cause its been 2 months with no injuries. Knocked on wood.

I regularly lose my balance doing BB lunges, and have fallen over once.
A few years ago, before I ever lifted, I was at the gym on the treadmill, running next to a beatiful girl. I let out a loud, wet, involuntary fart and immediately looked at the girl, hoping she had on headphones. She did not, and it smelled bad.

This. I was doing this in the corner standing (alt) leg and trying to get a few extra. Well, I go for it and all of a sudden lose my balance. I was able to throw the weight over head and there I went backwards to the ground. My legs were so exhausted I sat on the ground for about 30 secs. then popped up and looked the whole gym over. Then grabbed my towel and proceeded to my car. Leg day was over.Don't worry how others look focus on you and your goal. Workout for you and no one else.

Went to try to get heavy weight up on dumbbell press with no spotter. One arm went half way up, other didn't move. Weight fell on floor. LOL.

I was using the smith machine for standing barbell squats. I loaded way too much ****ing weight, 4 plates on each side and I wasn't able to balance right. The bar slid all the way down to the ground with my neck with it. It was like a guillotine like motion. Luckily the plates kept it from cutting my neck off.
